Please remember to inform the IoT of any changes during the year. This enables our registers to be updated, thus avoiding any delay with IoT correspondence/potential patient referrals from the website for you. Please complete both sides of the forms, even if your details have not changed.

Please enclose a copy of your current Business Insurance Certificate for Our Records. This can be an electronic scan- this is a condition of Membership and must be up to date and on record.
